As you consider a scenario in which you only had a few months left to live—what steps can you take to make them the very best they can be for you, your loved ones, and your caregivers? All you need to start is a vision for how you would like to prepare for your transition This is not an advance directive; however, it is a thoughtful plan for you to experience comfort, completion, happiness, and fulfillment in the final months, weeks, and days of your life.
I will guide you through building and implementing your vision as we reflect on and explore the answers to these questions:
What is most important to you?
What do you value most?
What comforting care do you want and need?
What are the things you can’t live without?
What priorities, wishes, and dreams do you want to focus on?
Who do you want to help fulfill your EOL Vision Map plan: friends, family, doctors, mentors, members of clergy?
We will delve into these questions in five domains of your life:
Physical: creating comfort, safety, and a healing environment
Emotional: honoring feelings and relationships
Spiritual: honoring beliefs and rituals
Mental: legacy life review
Practical: honoring the body after death
In the end you will have a vision map to which you can refer throughout for final journey.
